Baxter Regional Health System has acquired a Harrison medical clinic.
Effective Nov. 1, Washington Regional Crossroads Medical Clinic will become Baxter Regional Crossroads Medical Clinic.
The clinic will remain in its same location at 1420 U.S. Highway 62-65 in Harrison.
Current providers Victor S. Chu, MD; Kevin T. Jackson, MD; Jason Froeschle, APRN, Amanda Winford, APRN, as well as new provider Kim Cudworth, APRN, will join the Baxter Regional family.
To prevent any disruption in care during this transition, patients should complete and sign an Authorization to Release Medical Information Form. It is recommended that patients complete one of the following options to request that medical records are transferred to Baxter Regional Crossroads Medical Clinic:
• Stop by Crossroads Medical Clinic to sign a form;
• Call (870) 741-6111 to request a form via mail or email; or
• Visit www.baxterregional.org/crossroads to download a form.
